Please read the waiver below. It will be necessary for all youth who would like to participate in our clinics to provide us with a waiver signed by their parent or guardian. Please use the links on the left to download a printable copy of this form and ask the youth who you're representing to submit it to us upon their arrival at the clinic. If you would like us to mail you a copy, please contact us and be sure to give your mailing address. We appreciate your help!

Release, Waiver of Liability and Indemnity Agreement
In consideration of ____________________________ (hereinafter, the "Minor") being permitted to participate in TennisForKids.org sanctioned activities, the undersigned, as the Minor"s legal guardian (hereinafter "I"), for himself or herself, and on behalf of any personal representative, heirs, and next of kin, and on behalf of the Minor agree to the following:

1. WAIVER - I agree to release and waive TennisForKids.org, its directors, officers, employees, volunteers, and agents from all liability for any loss or damage, and any claim as a result of injury to the Minor's person or property (including without limitation the loss or theft of personal articles) or resulting in the Minor's death, whether caused by TennisForKids.org's negligence or otherwise while the Minor is participating in any TennisForKids.org sanctioned activity. In addition, I promise not to sue or otherwise seek compensation from TennisForKids.org, its directors, officers, employees, volunteers, and agents for any claims that may arise out of Minor's participation in TennisForKids.org sanctioned activities. In addition, I agree that Minor's photo and identity may be used to promote TennisForKids.org, including without limitation the posting of Minor's photo on the web site.

2. ACCEPTANCE OF RISK - I agree to assume full responsibility for and risk of bodily injury, death or property damage due to the negligence of TennisForKids.org or otherwise while Minor is participating in any TennisForKids.org sanctioned activity. I further acknowledge that tennis is an activity which could result in property damage or Minor's bodily injury or death.

3. NO FEES - I understand that TennisForKids.org is not charging me, nor Minor any fees or costs associated with TennisForKids.org sanctioned activities.

The undersigned further agrees that the foregoing RELEASE, WAIVER OF LIABILITY AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permissible under California law and that, if any portion is held invalid, I agree that the remainder of the provisions shall continue in full legal force and effect. My signature below affirms that I have read, understand, and voluntarily sign the RELEASE, WAIVER OF LIABILTY AND INDEMNITY AGREEMENT, and further agree that no oral representations or statements apart from the foregoing written agreement have been made.
